Physical Therapist Assistant (PTA) - Wyoming, MN Blue Stone Therapy is seeking a PRN Physical Therapist Assistant (PTA) to provide rehabilitation services to adult and geriatric patients at Meadows on Fairview in Wyoming, MN . This role focuses on delivering skilled physical therapy services to adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The Physical Therapy Assistant works under the supervision of a licensed Physical Therapist to implement treatment plans designed to improve mobility, strength, balance, endurance, pain management, and overall functional independence.
Requirements
- Graduate of an accredited Physical Therapy Assistant program
- Current Physical Therapy Assistant license (or eligibility for licensure) in applicable state
- Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred
- Strong communication, organization, and clinical reasoning skills
- Commitment to patient-centered care and professional standards

Responsibilities
- Provide skilled physical therapy treatment in accordance with established plans of care
- Implement individualized treatment programs focused on functional and measurable outcomes
- Treat adult and geriatric patients in a skilled nursing facility (SNF) setting and rehabilitation settings
- Monitor patient response to treatment and communicate progress with the supervising Physical Therapist
- Maintain timely, accurate documentation to support quality care and reimbursement standards
- Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes
