About The Position

Covenant Home Health Agency is seeking a dedicated and compassionate full-time Physical Therapist Assistant (PTA) to provide exceptional care to senior residents within the West Valley and Central Phoenix Senior Living Communities. Prior home care experience is required. Coverage area: Phoenix, West & Central Valley

Requirements

• Licensed Physical Therapist Assistant (PTA) in the state of Arizona.
• Current CPR certification.
• Previous home health experience is required.
• Excellent observation, verbal and written communication skills, problem solving skills, and basic math skills.
• Ability to work independently and as part of a collaborative team
• Compassionate, patient, and dedicated to improving the quality of life for senior patients.
• Must possess a valid driver’s license, reliable transportation, and the ability to travel throughout the West Valley and Central Phoenix areas.

Responsibilities

• Provide physical therapy treatments under the direction of a licensed Physical Therapist and in accordance with the care plan.
• Assist patients with therapeutic exercises, mobility training, and use of assistive devices.
• Apply physical therapy modalities such as heat, cold, electrical stimulation, ultrasound, and massage.
• Monitor and report patient responses and changes in condition.
• Maintain accurate and timely clinical documentation.
• Educate patients on proper use of mobility aids and equipment.
• Participate in team meetings and in-service training.
• Perform other duties as assigned.
