Optage is the Home & Community based division of PHS and is seeking a Full Time Physical Therapist Assistant in the East Metro. SCHEDULE/HOURS: Monday - Friday WHY BECOME A THERAPIST WITH PHS? Requirements

  • Graduate for an accredited program for physical therapy assistants.
  • Current licensure in the state of practice as required
  • Competence in physical therapy treatment and documentation according to the Standards of Practice
  • Demonstrated compatibility with PHS's mission and operating philosophies.
  • Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.

Responsibilities

  • Provide treatment per the established plan of care and in accordance with physician orders.
  • Assist in ide ntifying goals, prioritizing treatment, and selecting appropriate interventions .
  • Continuously assess changes in functional deficits and adjust treatment as appropriate .
  • P rovide education to residents /clients , families, caregivers, and staff.
  • Collaborate with therapist o n observations, progress towards goals, and recommended updates to plan of care and discharge.
  • Provide accurate and timely documentation & billing in electronic medical record.
  • Demonstrate understanding and knowledge of payer sources and regulatory requirements.
  • Coordinate with therapist on supervisory visit s per regulations .
  • Communicate effectively with residents/clients, families, caregivers, physicians, referral sources, and interdisciplinary team .
