About The Position

Are you passionate about making a meaningful difference in the lives of patients? At the VNA of Hanover & Spring Grove, we are looking for a compassionate and skilled Physical Therapist Assistant (PTA) to join our dedicated home care team. As a PTA, you will play a vital role in helping home health patients regain their independence and improve their quality of life through personalized physical therapy programs. Position Overview: The Physical Therapist Assistant will provide rehabilitative care for home health patients, assisting them with ambulation, transferring, gait training, and home exercise programs. Working under the supervision of a licensed Physical Therapist, you will be an essential part of our home care team, collaborating with RNs, OTs, SLPs, MSWs, and Home Health/Hospice Aides to ensure comprehensive, coordinated care.

Requirements

  • Graduate of a CAPTE-approved Physical Therapist Assistant program.
  • Successful completion of the PTA certification exam.
  • Active PTA certification in the Commonwealth of Pennsylvania.
  • Minimum of 2,000 hours of experience providing patient-related services under the supervision of a licensed Physical Therapist (home health experience preferred).
  • Current CPR certification.

Responsibilities

  • Deliver Compassionate Care: Provide physical therapy services according to a personalized care plan under the supervision of a licensed Physical Therapist.
  • Assess and Support: Help evaluate muscle strength, mobility, gait, and range of motion while assisting with equipment needs in the patient’s home environment.
  • Empower Patients and Families: Educate patients, families, and caregivers on therapeutic exercises and the use of durable medical equipment to promote recovery and independence.
  • Collaborate: Work closely with the Physical Therapist, attending physician, and the home care team to ensure each patient receives comprehensive, coordinated care.
  • Document Progress: Submit timely and accurate clinical notes in line with Medicare Home Health Guidelines and VNA policies.
  • Continuous Learning: Participate in patient case conferences, team meetings, and ongoing training to stay at the forefront of home health care best practices.
