About The Position

Haven Home Health is seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our growing home health team in North Phoenix and the surrounding areas . This is a full-time position providing one-on-one patient care in the comfort of their homes. As a PTA with Haven, you'll work closely with licensed Physical Therapists to carry out treatment plans and help patients regain their independence and mobility. Requirements

• Current Florida license as a Physical Therapist Assistant (PTA)
• Previous experience in home health preferred but not required
• Strong communication and time management skills
• Reliable transportation and valid driver’s license

Responsibilities

• Provide in-home physical therapy services under the direction of a licensed Physical Therapist
• Assist in implementing care plans and therapy goals
• Monitor patient progress and report changes to the PT and care team
• Educate patients and caregivers on exercises and mobility techniques
• Ensure accurate documentation in accordance with home health regulations
