Job Summary: ● Ongoing interdisciplinary assessment of the patient. ● Provides skilled therapy services per physician orders, the patient’s plan of care, agency policies and professional standards. ● Performs all aspects of patient/client care in an environment that optimizes safety. ● Ensures that changes in patients’ clinical status are communicated to other healthcare team members and agency office staff in a timely manner. ● Coordinate patient care and participate in case conferences to discuss multidisciplinary team patient progress, plans for continued care, new problems, etc. ● Educates and counsels patients, their families/caregivers, and other staff as appropriate, in appropriate speech therapy modalities. ● Provides, modifies, or discontinues physical therapy modalities based upon an ongoing assessment/reassessment of the patient’s clinical status and in accordance with physician orders ● Documents all care and services provided according to professional standards and agency policy and procedures. ● Instructs patients in proper in-home exercises and therapies. ● Educates patients and their families about the recovery process and the challenges they will face. ● Identify and meet patients’ goals and needs ● Provide quality, personalized and evidence-based care and proven interventions ● Promote clients’ healthy lifestyle by improving strength, flexibility, balance, and coordination

Requirements

Proven work experience as a Physical Therapist Assistant

Two years’ experience as a Physical Therapist Assistant
Current CPR Certification
Must maintain current driver’s license and auto insurance

Previous experience in the home care or outpatient therapy industry preferred.
