Your Role: Home Health Physical Therapist Assistant As a Physical Therapist Assistant in a home health setting , you'll work one-on-one with patients to restore mobility, reduce pain, and prevent falls, empowering them to live safely and independently at home.
Requirements
- California-licensed Physical Therapist Assistant (PTA)
- CPR certification (AHA-approved)
- At least 1 year of clinical experience (Home Health experience preferred)
- Organized, compassionate, and reliable
- Valid driver’s license, auto insurance , and reliable transportation
- Strong communication skills and clinical decision-making
Nice To Haves
- Home Health experience preferred
Responsibilities
- Provide in-home therapeutic care under the direction and supervision of a licensed Physical Therapist, following the established Plan of Care.
- Implement therapeutic exercises, balance training, and mobility interventions to help patients improve function and achieve their rehabilitation goals.
- Monitor and document patient progress , reporting changes in condition, functional status, or response to treatment to the supervising Physical Therapist and clinical team.
- Educate patients and caregivers on safe mobility techniques, home exercise programs, use of assistive devices, and fall-prevention strategies.
- Document visits using WellSky Kinnser EMR
- Coordinate care with physicians, RNs, case managers, and the interdisciplinary team
