Your Role: Home Health Physical Therapist Assistant As a Physical Therapist Assistant in a home health setting , you'll work one-on-one with patients to restore mobility, reduce pain, and prevent falls, empowering them to live safely and independently at home.

Requirements

  • California-licensed Physical Therapist Assistant (PTA)
  • CPR certification (AHA-approved)
  • At least 1 year of clinical experience (Home Health experience preferred)
  • Organized, compassionate, and reliable
  • Valid driver’s license, auto insurance , and reliable transportation
  • Strong communication skills and clinical decision-making

Nice To Haves

  • Home Health experience preferred

Responsibilities

  • Provide in-home therapeutic care under the direction and supervision of a licensed Physical Therapist, following the established Plan of Care.
  • Implement therapeutic exercises, balance training, and mobility interventions to help patients improve function and achieve their rehabilitation goals.
  • Monitor and document patient progress , reporting changes in condition, functional status, or response to treatment to the supervising Physical Therapist and clinical team.
  • Educate patients and caregivers on safe mobility techniques, home exercise programs, use of assistive devices, and fall-prevention strategies.
  • Document visits using WellSky Kinnser EMR
  • Coordinate care with physicians, RNs, case managers, and the interdisciplinary team
