About The Position

Transition Home Healthcare is an elite home health agency serving Houston, TX and surrounding areas since 2010. They are a group of passionate people dedicated to delivering the highest quality services to their patients, families, team members, and the community. The team is growing and looking for a dedicated Physical Therapist Assistant (PTA).

Requirements

• Assistant Physical Therapist in the state of Texas.
• Knowledge of home health regulations and general healthcare compliance.
• Able to handle physical workload.
• Good driving record.
• Excellent communication and people skills to effectively interact with patients, families, and healthcare team members.

Nice To Haves

• Home health experience highly preferred.
• Experience with HCHB EMR. (preferred but not required)

Responsibilities

• Assist in the facilitation of rehabilitation of patients with emotional, physical or mental disabilities.
• Assist in the implementation of physical therapy care plans.
• Assist with the coordination of care with the home healthcare team.
• Educate patients and family members about health conditions.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members.
