Physical Therapist Assistant (PTA) - [Laurens, IA] Blue Stone Therapy is seeking a Physical Therapist Assistant (PTA) to provide rehabilitation services to adult and geriatric patients at [Laurens Care Center] in [Laurens, IA]. This role focuses on delivering skilled physical therapy services to adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The Physical Therapy Assistant works under the supervision of a licensed Physical Therapist to implement treatment plans designed to improve mobility, strength, balance, endurance, pain management, and overall functional independence. This position will serve multiple partner communities, including [Fonda Specialty Care] and [Accura Healthcare of Pomeroy], with travel between locations as needed.

Requirements

  • Graduate of an accredited Physical Therapy Assistant program
  • Current Physical Therapy Assistant license (or eligibility for licensure) in applicable state
  • Strong communication, organization, and clinical reasoning skills
  • Commitment to patient-centered care and professional standards

Nice To Haves

  • Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred

Responsibilities

  • Provide skilled physical therapy treatment in accordance with established plans of care
  • Implement individualized treatment programs focused on functional and measurable outcomes
  • Treat adult and geriatric patients in a skilled nursing facility (SNF) setting and rehabilitation settings
  • Monitor patient response to treatment and communicate progress with the supervising Physical Therapist
  • Maintain timely, accurate documentation to support quality care and reimbursement standards
  • Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes
