Location This position is based in Crestview, FL, supporting Covenant Home Health Care patients through our partnership with Concierge Home Care. Your Role as a Home Health Physical Therapist Assistant (PTA) As a Home Health PTA, you will deliver high-quality, hands-on therapeutic care directly in patients’ homes—helping them regain mobility, maintain independence, and heal safely. Working under the supervision of a licensed Physical Therapist, you will play a vital role in implementing individualized plans of care. Requirements

  • Unencumbered Florida Physical Therapist Assistant (PTA) license (required)
  • Valid driver’s license, auto insurance, and reliable transportation (required)

Nice To Haves

  • Home health experience preferred, but not required

Responsibilities

  • Delivering therapeutic interventions in accordance with the Physical Therapist’s evaluation and plan of care
  • Assisting patients with mobility, strength, balance, and functional activities to promote safe recovery
  • Collaborating with patients, caregivers, Covenant leadership, and a supportive Concierge clinical team
  • Observing, documenting, and reporting patient responses and progress to the supervising Physical Therapist
  • Utilizing dictation services to streamline documentation and maximize patient-facing time
  • Educating patients and caregivers on exercises, safety techniques, and functional mobility strategies
