Location This position is based in Crestview, FL, supporting Covenant Home Health Care patients through our partnership with Concierge Home Care. Your Role as a Home Health Physical Therapist Assistant (PTA) As a Home Health PTA, you will deliver high-quality, hands-on therapeutic care directly in patients’ homes—helping them regain mobility, maintain independence, and heal safely. Working under the supervision of a licensed Physical Therapist, you will play a vital role in implementing individualized plans of care. Requirements

Unencumbered Florida Physical Therapist Assistant (PTA) license (required)
Valid driver’s license, auto insurance, and reliable transportation (required)

Nice To Haves

Home health experience preferred, but not required

Responsibilities

Delivering therapeutic interventions in accordance with the Physical Therapist’s evaluation and plan of care
Assisting patients with mobility, strength, balance, and functional activities to promote safe recovery
Collaborating with patients, caregivers, Covenant leadership, and a supportive Concierge clinical team
Observing, documenting, and reporting patient responses and progress to the supervising Physical Therapist
Utilizing dictation services to streamline documentation and maximize patient-facing time
Educating patients and caregivers on exercises, safety techniques, and functional mobility strategies
