Requirements

  • Associate Degree from an accredited Physical Therapist Assistant (PTA) program
  • Active Texas Physical Therapist Assistant (PTA) license
  • Home health physical therapy experience preferred (1+ year preferred)
  • Knowledge of Medicare home health documentation standards preferred
  • Current CPR certification
  • Strong communication, clinical judgment, and time-management skills
  • Valid Texas driver’s license
  • Reliable transportation and current auto insurance

Responsibilities

  • Provide skilled home health physical therapy interventions under the supervision of a licensed Physical Therapist (PT)
  • Implement and carry out individualized Plans of Care (POC) developed and delegated by the supervising PT
  • Treat patients with orthopedic, neurological, cardiac, post-surgical, or other medical conditions
  • Administer therapeutic exercises, gait training, balance training, transfer training, and functional mobility activities
  • Apply manual therapy techniques and non-complex modalities including heat, cold, electrical stimulation, and other physician-ordered treatments
  • Educate patients and caregivers on home exercise programs (HEP), fall prevention, and safe mobility strategies
  • Monitor patient response to treatment and promptly report changes in condition to the supervising Physical Therapist
  • Complete accurate and timely electronic documentation in compliance with Medicare home health regulations
  • Follow infection control standards and home health safety protocols
  • Participate in QAPI and agency performance improvement initiatives
