Documentation Mastery: Maintain accurate daily treatment notes and weekly progress reports in full compliance with California PT Board requirements. Interdisciplinary Teamwork: Participate in Patient Care and Rehabilitation Conferences to coordinate holistic resident recovery. Patient Advocacy: Maintain open and effective communication with the healthcare team regarding patient progress, challenges, and goals. Education & Support: Assist in training Restorative Aides and provide vital caregiver training and maintenance program instruction to families for safe discharge.
