Requirements

• Current PTA license in the state of Nebraska (or eligibility)
• Strong communication skills and ability to work independently
• A passion for providing patient-centered care in a home environment
• Reliable transportation and willingness to travel locally

Nice To Haves

• Experience in home health, outpatient, or rehab setting preferred

Responsibilities

• Provide high-quality therapy services under the direction of the Physical Therapist
• Support patients in achieving their mobility and functional goals through individualized treatment plans
• Deliver care in patient homes with professionalism, empathy, and strong clinical judgment
• Document visits thoroughly and timely in accordance with regulatory and company standards
• Partner with the care team to ensure exceptional outcomes for each patient
• Educate patients and families on exercises, safety, and care plans
