About The Position

Wolf Creek Care Center is seeking a dedicated Physical Therapy Assistant (PTA) to join their dynamic therapy team. The center is committed to providing high-quality, resident-centered care in a supportive and collaborative skilled nursing environment. Full-time, part-time, and PRN schedules are available. As a PTA, the individual will work hands-on with patients to implement physical therapy plans designed by licensed Physical Therapists, playing a key role in helping patients recover from illness, surgery, or injury, and promoting mobility, function, and quality of life. Requirements

• Associate degree from an accredited Physical Therapist Assistant program
• Active California PTA license
• Strong communication, teamwork, and documentation skills.

Nice To Haves

• Prior SNF experience preferred

Responsibilities

• Provide direct patient care under the supervision of a licensed Physical Therapist
• Document patient progress and communicate updates to the therapy team
• Collaborate with nursing, therapy, and medical staff to ensure coordinated care
• Ensure compliance with safety, infection control, and facility policies
• Maintain compliance with state and federal regulations and professional standards.
