About The Position

H2 Health Mobile Outpatient - 1:1 Patient Care 10-15 visits/week. Are you ready to make a difference in the lives of your patients? If you are passionate about patient-centered care and delivering high-quality outcomes, we want to hear from you! This position will treat a primarily geriatric caseload 1:1 within luxury senior living communities as well as residential visits within a 10-mile radius. You will work with a mix of general orthopedic conditions, vestibular rehab, neurologic movement disorders, and performance enhancement programs. The therapist must be able to work independently and in teams and make appropriate decisions based on each patient's functional needs.

Requirements

• Graduate of an accredited Physical Therapist Assistant program.
• Licensed Physical Therapist Assistant in the state of FL.
• Prior experience in outpatient Physical Therapy is preferred, but new grads are welcome.

Responsibilities

• Provide 1:1 patient care primarily to geriatric patients.
• Conduct residential visits within a 10-mile radius.
• Treat a mix of general orthopedic conditions, vestibular rehab, neurologic movement disorders, and performance enhancement programs.
• Work independently and as part of a team.
• Make appropriate decisions based on each patient's functional needs.
