About The Position

Join us as a Physical Therapist Assistant providing compassionate, expert care in patients’ homes! Mass General Brigham Home Care provides non-acute, medically necessary skilled care in the home to help “homebound” patients recover from illness or injury and manage exacerbations of chronic disease. Services include skilled nursing care, physical therapy, occupational therapy, speech language pathology, medical social services, and as needed medical supplies. We are seeking a full-time, 36-hour PTA to join the “Lighthouse” team. The entire team supports the following territories, Beverly, Marblehead, Prides Crossing, Salem and Swampscott. The desired schedule is Monday through Friday. Anticipated daily volume is 6-7 visits per day plus chart review, visit preparation, documentation, and care coordination. Job Summary Provide physical therapy services, under the direction of a registered physical therapist to patients in their place of residence, who have physical impairments, resulting from injury, disease, disability or other causes. Patients may range in age from birth to over 100 years. Collaborate with patient, family, physician and other allied health professionals to identify patient needs and coordinate services to meet those needs.

Requirements

  • Two years experience in acute care, rehab or homecare as a Physical Therapy Assistant
  • Licensed as a Physical Therapist Assistant in Massachusetts
  • Proficient computer skills and ability to learn other software applications
  • Strong verbal, written and electronic communications skills
  • Travel required within Mass General Brigham Healthcare at Home's geographic area
  • Ability to work with various computer software required and knowledge of hand-held patient care computer devices

Responsibilities

  • Provide physical therapy services, under the direction of a registered physical therapist to patients in their place of residence, who have physical impairments, resulting from injury, disease, disability or other causes.
  • Collaborate with patient, family, physician and other allied health professionals to identify patient needs and coordinate services to meet those needs.
  • Daily documentation is thorough and timely

  • Teaching is provided to patients and caregivers
