Requirements

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Rehab Therapist specializing in Physical Therapy (RPT) credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date required. A valid license pending letter will be accepted until license is issued.
• Bachelor's degree required.
• Master's degree preferred.

Nice To Haves

• 1-year acute care experience.

Responsibilities

• Evaluate patients with physical, emotional, cognitive, and/or psychosocial deficits.
• Design and implement patient-centered, effective treatment plans using appropriate modalities.
• Assess the patient's ability to safely perform independent living skills.
• Document patient progress, monitor, and modify the plan of care as necessary.
• Plan and assist with discharge, including education and recommendations for adaptive equipment.
