About The Position

Eddy SeniorCare is an exciting, innovative model of care for seniors, located in Rotterdam and Latham. We are a Program of All-Inclusive Care for the Elderly (PACE)--a unique model that is both the Medicare/Medicaid insurer and healthcare provider--giving us maximum ability to customize services to meet participant needs. As an insurer, we are at full risk, with the ability to combine Medicare and Medicaid resources to provide care in creative ways to reduce hospitalizations and avoid/delay nursing home placement. We care for 330 seniors, age 55 yo and above who have needs for long term support in the community. As a healthcare provider, we operate a primary care clinic, rehab clinic, a licensed home care agency, transportation service, adult daycare program, home delivered meals, activities, RN case management, social work, and more! We provide a team-based model of care including a physician, RNs, LPNs, Home Health Aides, social workers, physical/occupation/speech therapists, transportation, a day center team, and more! This is a truly unique program our region is fortunate to have access to, with only 130 PACE programs across the country! Rehab Supervisor –(Physical Therapy or Occupational Therapy) - Full Time Eddy SeniorCare PACE (A Program of All-Inclusive Care for the Elderly)– Schenectady & Latham, NY Eddy SeniorCare, a Program of All-Inclusive Care for the Elderly (PACE), is seeking an enthusiastic and compassionate Rehab Supervisor to lead our Occupational Therapy and Physical Therapy teams. This role is perfect for a clinician who loves mentoring others, enjoys a fast-paced interdisciplinary environment, and wants to use their skills to support some of the most vulnerable older adults in our community. What you will do: The Rehab Supervisor oversees the day to day operations of the Occupational Therapy and Physical Therapy departments at Eddy SeniorCare. You will coordinate and evaluate therapy services across our Day Center and participants’ homes, ensuring high quality care, strong team performance, and exceptional outcomes. You’ll provide both leadership and direct clinical care, helping participants reach their highest level of functional independence through an interdisciplinary, participant centered approach.

Requirements

  • Graduate of an accredited Occupational Therapy or Physical Therapy program
  • NYS Physical Therapist or Occupational Therapy Licensure
  • Minimum 2 years of OT or PT experience in acute care, rehabilitation, or home care.
  • Ability to work effectively with frail or disabled older adults.
  • Strong clinical judgment, flexibility, and excellent interpersonal skills.
  • Proficiency in documentation, EMR use, and interdisciplinary communication.
  • Ability to drive short distances for home visits when needed.

Nice To Haves

  • Previous supervisory experience preferred (but strong leadership qualities are just as valuable).

Responsibilities

  • Supervise, mentor, and evaluate all rehabilitation staff.
  • Coordinate therapy services, set priorities, and support staff productivity.
  • Ensure therapy programs in the Day Center are meaningful, engaging, and achieving intended outcomes.
  • Conduct chart reviews and participate in quality improvement initiatives.
  • Collaborate with physicians, nursing, social work, home health aides, and external agencies.
  • Ensure accurate documentation, timely visit notes, and compliance with regulatory standards.
  • Participate in recruitment, onboarding, education, and ongoing development of therapy staff.
  • Maintain strong communication with families and the entire care team.
  • Support safe, efficient use of equipment and monitor DME needs and tracking.
