Requirements

Applicants must hold an active Indiana license in their respective discipline (Physical Therapist, Physical Therapist Assistant, Occupational Therapist, Occupational Therapy Assistant, or Speech-Language Pathologist), or be eligible to obtain Indiana licensure.
Eligibility includes successful completion of an accredited educational program appropriate to the discipline, passing the required national and/or state licensure examination(s), and meeting all [state] licensure requirements.
