Requirements

• Current PTCB (Pharmacy Technician Certification Board) certification or Licensed Practical Nurse.
• High School graduate or equivalent.

Responsibilities

• Obtain and update patient medication allergy information in the electronic medical record.
• Review home medication list for duplications and/or omissions prior to entering into the patient's electronic medical record.
• Participate in committees or teams that are relevant to the practice area and medication reconciliation.
