The Harborview Medical Center Pharmacy Department is looking for an outstanding is looking for an outstanding Pharmacy Technician – Medication Reconciliation to join our Pharmacy team! As the region's only Level I Trauma and verified burn center, Harborview Medical Center is a comprehensive healthcare facility owned by King County and operated by UW Medicine. Requirements
- Six months of experience as a Pharmacy Assistant OR equivalent education/experience
- State of Washington Pharmacy Technician certification

Responsibilities
- Assist the Pharmacist in professional activities and assigned tasks including engaging in producing and recording an accurate patient medication list and other pharmacy related functions
- Obtain accurate home medication lists from admitted patients primarily through in person patient interviews in the Emergency Room (ER) and inpatient rooms, insurance claims and pharmacy phone calls.
- Accurately electronically transcribe home medication lists into the electronic health record.
- Escalate identified patient medication list concerns to pharmacist when needed.
- Answer inquiries not requiring a pharmacist’s professional judgement.
- Assist in the training of pharmacy technicians, students, and interns.
- Participate in monthly outdate and unit inspection program.
- Perform related duties as assigned.
