The Harborview Medical Center Pharmacy Department is looking for an outstanding is looking for an outstanding Pharmacy Technician – Medication Reconciliation to join our Pharmacy team! Requirements

  • Six months of experience as a Pharmacy Assistant OR equivalent education/experience
  • State of Washington Pharmacy Technician certification

Responsibilities

  • Assist the Pharmacist in professional activities and assigned tasks including engaging in producing and recording an accurate patient medication list and other pharmacy related functions
  • Obtain accurate home medication lists from admitted patients primarily through in person patient interviews in the Emergency Room (ER) and inpatient rooms, insurance claims and pharmacy phone calls.
  • Accurately electronically transcribe home medication lists into the electronic health record.
  • Escalate identified patient medication list concerns to pharmacist when needed.
  • Answer inquiries not requiring a pharmacist’s professional judgement.
  • Assist in the training of pharmacy technicians, students, and interns.
  • Participate in monthly outdate and unit inspection program.
  • Perform related duties as assigned.
