Summary The Pharmacy Technician supports pharmacists, patients, and providers by ensuring accurate medication reconciliation for individuals within the Curana Health census. This reconciliation process is a key component of delivering personalized care to our Chronic Care Management residents and patients. Through this work, we verify the accuracy of each patient’s documented medication regimen in the EMR while also identifying potential drug interactions, dosage issues, and opportunities for de-prescribing.
