About The Position

Provides pharmacy services in a specialty area under the direct supervision of a licensed pharmacist/licensed independent practitioner. Demonstrates advanced technical skills and ownership for the work area. The Transplant Transitions of Care Specialist technician will work directly with the inpatient solid organ transplant team to coordinate care delivery to transplant patients within the medical center. Specific duties may include but are not limited to: medication histories and documentation within the electronic medical record, collaboration with outpatient and specialty pharmacy technicians and systems for timely processing and delivering of discharge medications, coordination of filling pillboxes at discharge with transplant patients, and assisting the ambulatory transplant clinic pharmacist. This position is being filled for the dayshift, Monday through Friday. Demonstrates the ability to competently work in general and specialized areas of pharmacy practice. Participates in the training of new employees. Fills and delivers medications under the supervision of a licensed pharmacist/licensed independent practitioner. Policies and procedures are followed consistently. Ensures adequate supplies are available for continuous patient care. Triages and escalates any questions or issues outside the pharmacy technician scope of practice to the staff pharmacist or supervisor as needed. Uses appropriate systems for processing of patient information. Ensures quality assurance is maintained according to departmental and legal requirements. Operates pharmacy equipment appropriately. In addition to the above job responsibilities, other duties may be assigned.

Requirements

  • High School Graduate or Equivalent.
  • 6 months minimum relevant experience required.
  • Commonwealth of Virginia Pharmacy Technician Registration required.
  • CPhT credential through National Healthcareer Association (NHA) or Pharmacy Technician Certification Board (PTCB) required.

Nice To Haves

  • Certification as CPhT through NHA or PTCB required by January 31, 2022 for team members hired prior to February 1, 2020.
