About The Position

This role is responsible for supporting the provision of safe, effective, and prompt medication therapy with the supervision of a pharmacist. This role provides support of centralized and decentralized medication-use systems, as well as ensure optimal delivery of medication therapy to patients with various disease states. This role proficiently provides direct patient-centered care and integrated pharmacy operational services in a decentralized practice setting in collaboration with Home Delivery Specialty Pharmacy and other health system ambulatory clinical and pharmacy teams. This role reviews the electronic medical record to collect any pertinent information needed to complete monthly patient refill review to support existing HDSP clinical pharmacist workflows. They also will assist with disease state patient calls and order entry. This role contacts the patient, pharmacist, physician’s office and pharmacy team members through multiple methods as appropriate (include telephone, in-basket messaging, and electronic medical record). This role must have aptitude for accomplishing goals with minimal supervision and working independently with efficiency and self-motivation. The ability and understanding of all aspects and services provided through Froedtert's HDSP Services is crucial for this role. Demonstration of collaborating and fostering relationships with providers to ensure patients’ access to therapy is a requirement for this role. Requirements

  • High School diploma or equivalent is required.
  • Application for Wisconsin registration as a Pharmacy Technician is required by hire date and must be obtained within 12 months of hire. Staff hired into this role prior to 3/31/23 must have applied for Wisconsin registration by 3/31/23 and have until 3/31/24 to obtain registration.
  • Certification as a Pharmacy Technician (CPhT) through passing the Pharmacy Technician Certification Boards exam (PTCE) or the National Healthcareer Associations exam (ExCPT) is required within 6 months of hire.

Nice To Haves

  • A minimum of 3 years experience as a pharmacy technician is preferred.
  • A minimum of 1 year formal supervisory experience is preferred.
  • Graduate of an accredited pharmacy technician training program is preferred.

Responsibilities

  • Understand clinical disease state, medications, and record details in the patient medical record
  • Appropriately escalates pharmacist-level interventions to HDSP disease state clinical pharmacist
  • Proactive involvement and communicates with pharmacy and multidisciplinary teams to improve patient care
  • Rotates through various clinical disease states to complete patient reviews, Epic in-basket messages, follow ups, Patient calls/re-calls, and support URAC/ACHC accreditation standard reviews and other clinical pharmacist tasks.
  • Prepares for, conducts, and documents accurate list of prescription and specialty medications for patients enrolled in pharmacy patient management programs
  • Demonstrates ability to communicate (written and verbal) barriers to medication history information, identifies discrepancies, attempts to resolve them between medication information sources, and reports these to the pharmacist.
  • Maintains up-to-date knowledge of Froedtert Health Outpatient Pharmacy Services (e.g., Medication Access, Specialty Pharmacy, and Retail Pharmacy) and promotes utilization of services to patients and other members of the healthcare team, particularly as it relates to specialty medication needs
  • Assists with enrollment in services when patients express interest
  • Participates in the training of new team members, including pharmacy technicians, students, and pharmacists
  • Other duties as assigned
