Under the direct supervision of a pharmacist, performs transition of care (TOC) related tasks such as collection and verification of allergy and medication history. The TOC liaison will communicate with patients, families, nurses, physicians, and other healthcare professionals to obtain accurate home medication lists upon hospital admission. Provides integrated support for patients and the hospital’s admitting team to ensure a safe medication transition by providing a “hands-on” approach to total quality patient care.

Requirements

  • High school diploma or equivalent required.
  • Massachusetts pharmacy technician registration and national pharmacy certification by Pharmacy Technician Certification Board (PTCB) required.
  • Three years pharmacy experience required, with both community and inpatient hospital experience preferred.
  • Must be proficient in math with knowledge and ability to use a computer for data entry.
  • Must be able to follow oral and written instructions with accuracy, efficiency, and dependability.
  • Strong interpersonal communication skills; strong phone skills; competent in Microsoft software; self-motivated; independent; strong organizational skills; high-integrity; keen attention to detail; dependable; empathetic; strong listener; energetic; team-player; values discretion and confidentiality; efficient.

Nice To Haves

  • Experience with medication history program preferred.
  • Community and/or ambulatory pharmacy experience preferred.

Responsibilities

  • Provides integrated support to patients, pharmacists, and various healthcare providers by providing a “hands-on” approach to total quality patient care.
  • Works closely with patients and/or their families to obtain the best possible medication and allergy history.
  • Includes: Interviewing patients and/or their families Utilizing active listening skills Utilizing available resources to overcome barriers (i.e. language barriers) Understanding basic medication information
  • Integrate and collaborate with the emergency department’s admitting clinical teams, consisting of physicians, nurse practitioners, nurses, and case managers (i.e. Transition of Care Team, ED Team, Medicine Team, Surgery Team,etc.).
  • Operate electronic health records to obtain patient medication history information.
  • Contact outside medical facilities to obtain patient medication history information. Includes: Pharmacies Physician offices Hospitals Long-term Care Facilities
