Requirements

• Pharmacy Technician License for the state of TN
• 1+ years of experience in sales, marketing, or customer service
• Ability to drive with an active driver's license and access to reliable transportation

Responsibilities

• Build solid relationships with patients, hospital staff, pharmacy team, and other stakeholders.
• Participate in daily/weekly/monthly team meetings and act on feedback to ensure quality care on-site.
• Educate potential patients about services provided at discharge and how to utilize pharmacy services.
• Coordinate the collection of prescriptions, patient orders, and necessary paperwork for medication dispensing.
• Schedule and coordinate pharmacist consultations to drive medication adherence and education.
• Handle coordination of patient medication delivery at discharge and ensure education is provided to caregivers.
• Identify and resolve insurance billing issues, including facilitating prior authorizations and assisting with financial assistance.
• Conduct weekly follow-up calls with hospital patients post-discharge to ensure aftercare plans are met.
• Identify trends and barriers, recommend innovative solutions, and propose value-driven changes.
