We are seeking a Pharmacy Technician Med Rec for our team to ensure that we continue to provide all patients with high quality, efficient care. The medication history pharmacy technician specialist performs medication histories for new hospital admission. Under general supervision of the clinical pharmacist, provide complete and accurate information for each medication, e.g., drug name, dose, route, frequency, indication, and date/time last dose was taken. Schedule may vary according to the needs of the facility/department and may include weekend, evening shift, holiday, on-call, and additional hours.

  • Performs medication histories
  • Interviews patients and family members for the purpose of compiling an accurate and complete list of current medications the patient was taking prior to admission to the hospital
  • Contacts the patient's community pharmacy, physician's office, or other healthcare facility for clarifications if needed
  • Accurately updates medication history, allergies, adverse drug reaction, pharmacy and primary care provider information in the electronic health record
  • Correctly and completely documents interventions made to the medication history in the electronic medical record prior to the end of each shift worked
  • Discusses any questions or issues that arise during the medication history process with a clinical pharmacy specialist
  • Appropriately communicates unresolved medication history issues to clinical pharmacy specialists
  • Documents patient medication histories in the electronic health record and CPW/VigiLanz
  • Ascertains need for language assistance and obtains interpreter as necessary
  • Assists the clinical pharmacist performing education and/or discharge counseling through identification of patients that need medication histories completed by pharmacy services and preparation of counseling materials
  • Maintains competence required for current position
  • Maintains pharmacy technician certification and Pharmacy Technician Registration
  • Attends and participates in staff meeting regularly
  • High school graduate or GED required
  • ≥ 2 years previous retail pharmacy experience required
  • ≥ 1 year previous inpatient pharmacy experience preferred
  • Maintains current pharmacy technician license or pharmacy intern license required
  • Board Certification as a Pharmacy Technician for registered pharmacy technicians preferred
