Benefits Eligible Full-Time Benefit Eligible Work Shift Variable - 8 Hour or less Shift (United States of America) Department 8290 Pharmacy-PGY1 Residency If assigned to PGY1: PGY1 pharmacy residency programs build on Doctor of Pharmacy (Pharm.D.) education and outcomes to contribute to the development of clinical pharmacists responsible for medication-related care of patients with a wide range of conditions, eligible for board certification, and eligible for postgraduate year two (PGY2) pharmacy residency training. Kaweah Health is committed to producing PGY1 trained clinical pharmacists that exhibit the core characteristics of leadership, evidenced-based practice and service to the community. The PGY1 residency will train pharmacists in professional practice and management activities where they will gain in-depth experience leading to advanced practice skills and knowledge of the integration of hospital pharmacy services with the comprehensive needs of individual practice settings. Upon completion, residents will be prepared to practice independently as a general clinical practitioner in an acute care setting or pursue further specialized training. If assigned to PGY2: PGY2 Program Purpose: PGY2 pharmacy residency programs build on Doctor of Pharmacy (Pharm.D.) education and PGY1 pharmacy residency programs to contribute to the development of clinical pharmacists in specialized areas of practice. PGY2 residencies provide residents with opportunities to function independently as practitioners by conceptualizing and integrating accumulated experience and knowledge and incorporating both into the provision of patient care or other advanced practice settings. Residents who successfully complete an accredited PGY2 pharmacy residency are prepared for advanced patient care, academic, or other specialized positions, along with board certification, if available.

Requirements

  • License /Certification Required: Current California Registered Pharmacist license or be eligible to be licensed as a pharmacist in the State of California. Licensure must be obtained by October 1st of the residency year. Current California Pharmacy Intern license by residency start date if California Pharmacist licensure has not already been obtained.
  • Education Required: Graduate from an ACPE-accredited Doctor of Pharmacy (PharmD) program.
  • Experience Required: If assigned to PGY2: current participation in, or completion of, an ASHP-accredited PGY1 pharmacy residency program or one in the ASHP accreditation process (i.e., candidate or preliminary accreditation status).
  • Knowledge/Skills/Abilities Requires the desire to be a highly motivated pharmacist wanting advanced education and training that leads to an enhanced level of professional pharmacy practice.
  • Department Specific Requirements Participation in the program's residency candidate review process including an on-site interview, and adherence to rules and regulations and participation in ASHP Resident Matching Program or Early Commitment process.

Responsibilities

  • Completes all learning activities established as part of the educational outcomes, goals and objectives for the residency program within the allotted time frame.
  • Enhances pharmaceutical care by contributing value-added services to patients.
  • Defines and completes required projects as required by the residency program: major research, community service and formulary.
  • Actively contributes to the development, implementation and monitoring of departmental operations with department managers.
  • Promotes institutional dissemination of information to other members of the health care team to promote quality pharmaceutical services.
  • Actively participates in pharmacy related education activities that involve mentoring of interns, externs and in-service training of pharmacy and non-pharmacy personnel.
  • Participates in departmental recruitment and interview process for staff and residents.
  • Performs the functions of a pharmacist with applicable oversight as defined by the program staffing requirements and in conjunction with assigned patient care activities and licensure.
  • IF ASSIGNED TO PGY1, WHEN ASSIGNED TO PERFORM PHARMACIST RESPONSIBILITIES: Dispenses medication, supplies or equipment orders accurately and efficiently.
  • Follows written policies and procedures regarding medication administration.
  • Consults with other health care providers and information resources to optimize therapy and maximize safety for medication administration for assigned areas.
  • Uses PYXIS units on the nursing units.
  • Responsible for answering all inquiries from physicians and other staff regarding medication administration issues.
  • Compounds IV medication and solutions accurately and efficiently.
  • Uses strict aseptic technique and maintains 100% accuracy in measurement and dose calculations when compounding.
  • Complies with OSHA guidelines and procedures.
  • Maintains absolute sterile technique in IV room at all times.
  • Guides, directs, and trains pharmacy techs and assumes responsibility for quantity and quality of their work.
  • Monitors patient's drug and nutritional therapies and maintains their clinical charts with complete and timely documentation.
  • Provides patient specific dosing and monitoring information.
  • Provides consultation and actively participates in patient treatment.
  • Offers cost effective and safe alternatives when appropriate.
  • If assigned to fill outpatient prescriptions, does so in accordance with federal and state law and in accordance with hospital policies Patients shall be counseled upon receipt of prescription on the appropriate use, storage and potential adverse reactions.
  • Responds to Code Blue alert as a top priority.
  • Makes up all dilutions, IV's, etc., ordered by the physician.
  • Anticipates patient drug needs and responds accordingly.
  • Fills all narcotic requisitions as presented by the techs after ascertaining correctness of requisition.
  • Maintains all appropriate records demanded by federal and state laws and KH procedures.
  • Conducts monthly nursing station inspections, checking for cleanliness, outdates, orderliness, and unauthorized drugs as required by TJC and Title 22 and keeps all appropriate records.
  • Organizes and plans monitoring of drugs encompassed by pharmacokinetics following all department procedures.
  • Consults with physician on optimal drug therapy.
  • Monitors patients following department procedures.
  • Performs antibiotic chart review following department procedure.
  • Generates written reports.
  • Provides information to modify therapies if required.
  • Prices patient charges the day they accrue under direction of manager.
  • IF ASSIGNED TO PGY2, WHEN ASSIGNED TO PERFORM PHARMACIST RESPONSIBILITIES: Serves as clinical expert in the area of ambulatory care pharmacy services providing pharmacotherapy education to physician and pharmacy residents, students, staff, faculty and patients.
  • Provides both consultative and direct patient care services for referred patients via collaborative practice agreements in the ambulatory clinic setting.
  • Recommends and/or orders pertinent labs and patient assessments for the purposes of monitoring and managing the pharmacotherapy regimens of patients to assess efficacy, safety and toxicity.
  • Provides in-person and telephonic Medication Therapy Management (MTM) services for identified patients that include Complete Medication Reviews and Targeted Medication Reviews to both patients and prescribers.
  • Reviews medication profiles for high-risk patients to ensure appropriateness of therapy with interventions to the prescriber when necessary.
  • Provides timely and effective communication with referring prescribers and allied healthcare staff regarding patient care activities according to Kaweah Health policy.
  • Participates in the development and implementation of proactive population management programs for high-risk patient populations.
  • Guides, directs and trains pharmacy technicians assigned as patient care managers to ambulatory care services.
  • Serves as an Ambulatory Care primary and co-preceptor for pharmacy residents and interns.
  • Ongoing participation in the planning and development of the integration of pharmacy and family medicine training programs to maximize synergy and promote interprofessional models of care.
  • Provides didactic lectures and facilitates patient case discussions related to pharmacotherapy to family physician and pharmacy residents, students, staff and faculty.
  • Identifies opportunities for improvement in the area of pharmacotherapy and provides education, training and clinical services.
  • Documents clinical, fiscal and educational interventions on an ongoing basis.
  • Measures, quantifies and qualifies the effectiveness of the services and interventions provided in terms of quality, clinical and economic improvements on an ongoing basis with formal reporting of the data at defined intervals set by management.
  • Demonstrates the knowledge and skills necessary to provide care and services appropriate to the population served on the assigned unit or work area.
  • Knowledgeable of growth and development for all patient/family cultural, linguistic, spiritual, gender, and age specific needs.
  • Able to effectively communicate and care for patient and family as reflected in the Plan for Provision of Care.
  • Performs other duties as assigned.
