Our client is leading Indiana’s 5-year Rural Health Transformation Program, including the Make Rural Indiana Healthy Again Regional Grants, a historic $600M investment to strengthen healthcare access, prevention, and system resilience across 64 fully rural and nine partially rural counties. This initiative brings together state leadership, healthcare providers, and community organizations to design and implement solutions that reflect local needs while advancing a shared, statewide vision for better health outcomes. This is a high-impact role focused on expanding access to critical medications for patients transitioning from hospital to home, helping reduce barriers to care and improve outcomes in rural communities across the state. About the Role As the Pharmacy Program Manager, you will support Growing Access to Hospital Post-Discharge Medications by helping rural hospitals implement and strengthen Meds to Beds programs across Indiana. In this role, you will work closely with the Indiana Department of Health, the Family and Social Services Administration – Office of Medicaid Policy and Planning, rural hospitals, and other stakeholders to support hospital-based medication access programs that ensure patients receive prescribed medications before discharge. You will help drive implementation, build partnerships, provide technical assistance, and support performance tracking to improve care transitions and reduce access and transportation barriers. This is a highly collaborative role that requires strong relationship-building, program coordination, and the ability to support hospitals and partners through implementation, problem-solving, and continuous improvement.
