About The Position

The Pharmacy Outreach Coordinator plays a vital role in expanding patient access to specialty pharmacy services. This position is responsible for proactively identifying and engaging eligible patients through outbound phone calls, with the goal of enrolling them in the Sentara Specialty Pharmacy Program. The coordinator will serve as a key liaison between patients and the pharmacy, providing education on the comprehensive services offered, including medication management, financial assistance, and personalized support. Conduct outbound phone calls to patients, explaining the value and advantages of the Sentara Specialty Pharmacy Program compared to other pharmacy care models. Access and review patient Electronic Medical Records (EMR) and chart notes before outreach to understand the patient’s condition, treatment plan, and medication needs. Identify medications that require special handling, storage, or administration (e.g., hazardous or refrigerated drugs). Accurately document all communications and actions in the patient record in compliance with HIPAA and organizational standards. Collaborate with clinical and pharmacy staff to support seamless patient onboarding and follow-up. Perform additional duties and administrative tasks as assigned.

Requirements

  • High School Diploma or Equivalent (Required)
  • Virginia Pharmacy Tech License (Required)
  • CPhT License (Received through the NHA or PTCB) (Required)
  • 2 Years of Pharmacy Experience (Required)
