The ambulatory pharmacy navigator program helps HMH Discharge and Specialty Pharmacy patients receive their discharge/specialty medications quickly and efficiently. This allows patients to be discharged from the hospital smoothly and begin their recovery at home or start their specialty medication therapy right away. The Navigator I plays a crucial role in ensuring patients receive their prescriptions smoothly. They gather all the necessary information, prioritize tasks, address any financial, insurance, or clinical challenges, and make sure the prescriptions are delivered to patients before they leave the hospital or directly to their homes. The Navigator ensures OBRA compliance by ensuring an offer to counsel is made and that patient medication educational needs are met. The Navigator I functions will include all job functions of an Ambulatory Pharmacy Technician plus additional duties described below.

Requirements

  • Experience as an Ambulatory Pharmacy Technician or similar role.
  • Knowledge of OBRA compliance and patient counseling requirements.
  • Ability to manage multiple tasks and prioritize effectively.
  • Strong communication skills to interact with patients and healthcare providers.

Nice To Haves

  • Previous experience in a hospital or healthcare setting.
  • Familiarity with specialty medications and discharge processes.

Responsibilities
