About The Position

The Pharmacy Medication Reconciliation Technician (PMRT) plays a crucial role in ensuring accurate medication management for patients admitted to Wooster Community Hospital. This position involves collecting and entering patient medication histories into the electronic health record (EHR), processing discharge prescriptions, and communicating any discrepancies to pharmacists or physicians. The PMRT also assists in medication management under the supervision of a pharmacist and is responsible for maintaining medication security and compliance within the Emergency Department.

Requirements

• High school diploma or GED
• 2-3 years of pharmacy technician experience
• Pharmacy Technician Certification
• Strong communication skills
• Basic typing and math skills
• Familiarity with medication names, doses, and allergic reactions
• Attention to detail and self-motivation

Nice To Haves

• Retail pharmacy technician experience preferred

Responsibilities

• Obtain a patient medication history by interviewing the patient and family members.
• Document the medication list accurately in the EHR, ensuring correct doses and schedules.
• Resolve discrepancies in the medication list by contacting physicians and pharmacies.
• Process discharge prescriptions for ED patients using the Retail Pharmacy Computer System.
• Maintain fluency in prescription insurance information processing.
• Interview patients to obtain accurate allergy information and document it in the EHR.
• Perform monthly inspections of the Emergency Department for medication expiration dates.
• Facilitate the restocking of ED kits and crash cart drug drawers.
• Ensure controlled substance medication discrepancies are resolved in a timely manner.
• Expedite delivery of critical medications to the Emergency Department.
