About Corewell Health Lakeland Retail Pharmacy Corewell Health Retail Pharmacy at Lakeland Hospital in St. Joseph is an open-door outpatient pharmacy located at St. Joseph Hospital. While this pharmacy provides traditional outpatient pharmacy services for the community, its main purpose is to operate a Discharge Medication Program to collaboratively work across professional disciplines to ensure patients discharging from the hospital leave with their new medications in hand. The newly added 22,000-square-foot facility in Niles, Michigan will have 43 exam rooms and will house both Southwestern Medical Primary Care, Niles and Spectrum Health Lakeland Ear, Nose, and Throat providers. The $7.8 million project is expected to be completed and open to patients March 2023. This facility is a continued investment in our Niles community and the health of those we serve. It houses a range of coordinated health services from diagnostics to primary and specialty care, all in one place, allowing us to deliver care to a patient’s whole family, right in their community. https://lakelandcare.com/directory/facility/corewell-health-lakeland-hospitals-st-joseph-hospital Scope of Work Under the direct supervision of a Pharmacist, formulates, prepares, and delivers medications and pharmaceutical supplies. Exercises professional judgment in the interpretation of orders. Trained in medication reconciliation duties and pharmacy technician skills. Works with multiple medication automation systems. Interprets physician’s orders checking for correct dose, drug, route, rate and frequency of administration. Reviews patient profile for allergies, duplication of drugs and drug interactions and accurately enters orders into computer under the supervision of a pharmacist. Aseptically reconstitutes, filters, prepares and labels parenteral, sterile products, and chemotherapy, and dispenses other medications under the supervision of a pharmacist. Maintains documentation and record keeping. Participates in patient care activities such as drug use monitoring, pharmacokinetics dosing service and nutritional support services. Provides drug information to physicians, nurses, and other members of the team in collaboration with a pharmacist. Participates in special projects, such as data entry or research. Completes all Spectrum Health assigned competencies by assigned due dates.
