Position Overview: We are seeking a motivated and dependable Pharmacy Extern to join our Meds2Beds Program on a weekend-only basis. This role supports timely medication delivery and patient education for those being discharged from the hospital, playing a key role in reducing readmissions and improving patient outcomes.
Requirements
- Enrolled in an accredited Pharmacy program.
- Holds a valid pharmacy intern/extern license.
- Current Basic Life Support (BLS) certification.
- Strong interpersonal and communication skills.
- Detail-oriented and committed to patient-centered care.
Responsibilities
- Deliver prescribed medications directly to discharged patients.
- Provide patient counseling on medication usage, potential side effects, and the importance of adherence.
- Collaborate with pharmacists, nurses, and care teams to ensure smooth patient transitions.
- Accurately document prescriptions and patient interactions in compliance with hospital and pharmacy protocols.
- Provide coverage during weekends and holidays as needed.
