Primary responsibility is to facilitate the development, implementation, and continuous quality improvement of coordinated pharmacy services across the system. Provides guidance for the integration of clinical pharmacy services, drug utilization programs, and pharmacy operations throughout the institution. Ensures medication use is aligned with evidence-based best practices and regulatory standards. Directs the activities of pharmacists who work within assigned area. Work to meet highly specialized pharmacy business needs in programs such as Clinical Services, Medication Safety, Antimicrobial Stewardship, Education and Staff Development, Supply Chain, Policy, Regulatory, Informatics, Operations Services. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all. This role will support the Executive Director of Post-Acute Pharmacy Practice in advancing strategic initiatives, aligning practice with evidence-based standards and organizational goals, across both inpatient and outpatient settings. Responsibilities include but are not limited to driving process improvement, service line support and coordination, conducting data analysis, developing protocols/policies, and education. The position requires strong leadership and collaboration with multidisciplinary teams to optimize workflows and coordinate efforts across inpatient and outpatient spacesincluding ambulatory clinics, specialty programs and transitions of care. A key component of this role involves supporting efforts to close gaps in patient care, ensuring continuity, safety, and quality across the care continuum
