A caregiver in this role will provide advanced clinical pharmacy services within Cleveland Clinic's Ambulatory Heart Failure Clinics. Practicing under a collaborative practice agreement, this pharmacist will independently initiate and adjust heart failure and complementary medication therapies. Key responsibilities include guideline-directed medical therapy (GDMT) initiation and titration, management of pre- and post-cardiac transplant patients, and GLP-1 therapy titration. The pharmacist will collaborate closely with cardiologists, transplant teams, and interdisciplinary clinic staff to optimize patient outcomes and advance the quality of care.
