Coordinates and provides the pharmaceutical care of patients for an assigned clinical unit and participates in patient care rounds with medical staff personnel. A caregiver in this role will provide advanced clinical pharmacy services to support patients with endocrine disorders through collaborative practice agreements. This caregiver will work closely with endocrinologists, primary care providers, diabetes educators, and interdisciplinary care teams to optimize medication therapy, enhance disease management, and improve patient outcomes. Responsibilities include initiating, adjusting, and discontinuing medications under consult agreements, conducting individual and group patient visits (in-person and virtual), interpreting continuous glucose monitoring data, managing insulin pumps, and delivering patient and provider education. The pharmacist will also support pharmacy services in specialty clinics, including post-kidney transplant and diabetes in pregnancy. A caregiver in this role will work full time, Monday - Friday from 8am-5pm.
