The Pharmacy Clinical Specialist – Emergency Medicine at Fairview Hospital practices in a decentralized clinical pharmacy model with active, on-site coverage in the Emergency Department. This role supports high-volume ED patient care through real-time medication therapy monitoring, clinical consults and direct collaboration with interdisciplinary care teams. Responsibilities include responding to urgent clinical needs, supporting inpatient acute care needs interfacing with the ED and providing medication therapy management with a strong focus on safety. The specialist applies advanced clinical decision-making, communicates effectively in high-acuity environments and demonstrates the ability to multitask and reprioritize in a fast-paced setting while supporting Emergency Medicine service lines and overall patient care. A caregiver in this position works Monday through Friday, either the morning shift from 7:00am to 5:00pm or night shift from 3:00pm to 1:00am. Requirements include every 3rd weekend from 8:30am to 9:00pm.

Requirements

  • PharmD or Bachelor’s Degree in Pharmacy from an accredited college of Pharmacy
  • Current state licensure as a Registered Pharmacist (RPh) or eligible within 90 days of hire
  • Current Basic Life Support (BLS) Certification through the American Heart Association (AHA) and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days of hire (if administering immunizations)
  • Completion of a Post-Graduate Year 2 Residency (PGY2) or fellowship OR completion of a Post-Graduate Year 1 Residency (PGY-1) and two years of experience as a Clinical Specialist or an equivalent role, including providing pharmaceutical care and performing pharmacotherapeutic monitoring for at least 50% of time in the specialty area OR four years of experience as a Clinical Specialist or an equivalent role, including providing pharmaceutical care and performing pharmacotherapeutic monitoring for at least 50% of time in the specialty area

Responsibilities

  • Routinely and continually provide pharmaceutical care for patients, considering their age and clinical condition.
  • Provide general and patient specific drug information to patients, physicians, nurses and, and other health care professionals.
  • Support Clinical Pharmacists in providing pharmaceutical care to patients by direct interaction and formal educational programs.
  • Maintain expertise in areas of practice through continual learning, journal review and other educational activities.
  • Educate Clinical Pharmacists, Technicians, Pharmacy Students, Post-Graduate Trainees and other caregivers in Pharmacotherapy within in the specialty area.
