Pharmacy Care Coordinator... Full-Time-1.0 FTE | 10 hr. shifts First-Shift BHCOR-Pharmacy I Corbin, KY The Pharmacy provides pharmaceutical services to patients across the continuum of care. Services include, but are not limited to verification of medication orders, drug distribution, sterile product preparation, hazardous drug preparation and a meds to beds program for inpatients. The Specialty Pharmacy Care Coordinator acts as an extension of the ambulatory clinic teams to support patient access to high touch, complex medication therapies. This position is based in a specialty pharmacy clinic setting under pharmacist supervision. The Specialty Pharmacy Care Coordinator functions in a multiskilled environment which requires the application of teamwork, leadership, and flexibility. Works in collaboration with insurance companies, nursing, and social workers to obtain appropriate authorization for medications including registration with drug specific manufacturer programs when applicable. Performs tasks related to prescription billing including completing benefits investigations, assisting with the prior authorization process, and locating copay assistance options for patients to ensure affordability of specialty medications. Assists with clinic intervention outcomes tracking to monitor value of services. Coordinates patient outreach for new and enrolled patients to ensure continuity of care and optimize patient outcomes. Job Description: Pharmacy Care Coordinator Verifies patient eligibility and coverage for specialty medications and stays current on payer requirements. Supports prior authorizations, obtains overrides, and prevents gaps in therapy by maintaining accurate insurance and renewal information. Triages clinical or insurance-related questions to pharmacists and documents all care activities in the EHR. Serves as the main contact for patients, providing compassionate support and coordinating specialty medication needs. Communicates effectively with providers and care teams regarding coverage changes, requirements, and patient needs. Assists patients with enrollment in financial assistance programs and gathers needed documentation. Uses technology to manage refills, delivery coordination, outreach tasks, and timely claim processing. Follows organizational policies, safety standards, and accreditation requirements, participating in needed training and preparation.
