The Pharmacist provides quality patient-oriented pharmaceutical care which includes providing drug information and consultation to physicians and other health care personnel. Responsibilities include receiving medication orders written by physicians, reviewing each for appropriate therapy, managing the dispensing of medications to the patient care areas, and monitoring patient response to drug therapy. The pharmacist documents activities in the medical record, ensuring that hospital policies and procedures are followed and documented, including that entries are signed, dated, timed, legible, and concise. Activities and recommended plans are clearly presented. The pharmacist manages drug distribution to patients and patient care areas, ensuring that distribution activities conform to standards of practice, appropriate safety practices, and hospital policies and procedures. It is essential that patients receive the correct medication in a timely manner. The pharmacist reviews and interprets medication orders, ensuring that orders are reviewed and validated in a timely manner and consistent with established guidelines. Therapy and dosage must be appropriate for diagnosis, and allergy conflicts, drug interactions, and incompatibilities are identified and resolved. The pharmacist also manages drug therapy by identifying drug therapy problems and ensuring that problem lists are complete. Drug therapy regimens and recommendations are designed to reflect best evidence, and appropriate monitoring plans are developed and implemented. Recommendations must be clearly presented, and protocols are managed in accordance with approved guidelines. The pharmacist responds to consults and provides expert recommendations, ensuring that responses are timely, accurate, complete, and presented in a professional manner.
