Job Description Schedule: M-F rotating mostly days, some evenings, nights and one weekend per month. JOB SUMMARY The Pharmacist is responsible for the delivery of direct, patient-centered care to optimize the safe, effective and efficient use of medications across each continuum of care. Pharmacists rotate through the dispensation roles; as well as, contributing to direct patient care on the units alongside the healthcare team.

Requirements

  • Kansas Pharmacist License
  • PGY1 residency or equivalent experience (3-5 years of inpatient, clinical pharmacy practice)
  • BLS Certification

Nice To Haves

  • ACLS Certification
  • Board Certification or other nationally recognized pharmacist training
  • Interest in mentoring and precepting students and residents

Responsibilities

  • Analyze and verify electronic orders.
  • Dispense medications according to policies and procedures.
  • Prepare or check compounded medications (from topical products to intravenous medications)
  • Respond to medication information questions from LMH Health staff, the community and our patients.
  • Complete other duties as needed or assigned by supervisor, such as serving as the lead pharmacist for designated unit(s) and corresponding committee meetings
  • Supervise, in conjunction with the technician supervisor, the technician and intern staff.
  • Obtain accurate medication histories and perform medication reconciliation on admission and discharge.
  • Identify therapeutic interventions and communicate effectively with the healthcare team to optimize the patient’s plan of care.
  • Monitor medications associated with the plan of care, including ordering necessary labs per protocol.
  • Respond to consults and follow collaborative practice agreements as approved by the Pharmacy & Therapeutics Committee.
  • Respond to Medication Evaluation Team and Code calls (Stroke, STEMI, Trauma, and Blue).
  • Maintain internal competencies as required by the pharmacy department and organization.
  • Complete biannual CE requirements to maintain state licensure.
  • Participate in educational opportunities throughout the health-system and community.
  • Support LMH Health’s purpose and commitment statements.
  • Precept pharmacy students on experiential rotations and residents completing a PGY1 residency.
