About The Position

Capital Cardiology Associates (CCA) is seeking a full-time clinical Pharmacist (PharmD) to work Monday through Friday. A clinical pharmacist at CCA is responsible for assuring optimal medication use outcomes in our cardiac patient population. A qualified candidate will possess strong written and verbal communication skills. This position will require skills to provide direct patient care in an efficient, fast-paced, changing environment.

Requirements

  • Doctorate of Pharmacy Degree from an ACPE accredited school of Pharmacy
  • Successful completion of ASHP-accredited PGY1 clinical pharmacy residency
  • PGY2 pharmacy residency in cardiology or other applicable specialty OR PGY1 residency plus 2 additional years of practice with at least 50% of time spent in direct patient care related specialty
  • Board Certification in Cardiology (BCCP) to be completed within 2 years of hire
  • CDTM to be obtained within 14 months of hire
  • Current, active, Pharmacist License required at hire
  • NYS license required within 3 months of hire
  • National Provider Identifier required at hire
  • Strong written and verbal communication skills
  • Skills to provide direct patient care in an efficient, fast-paced, changing environment

  • PGY2 pharmacy residency in cardiology
  • Cardio-metabolic related specialty for 2 additional years of practice with at least 50% of time spent in direct patient care

Responsibilities

  • Apply specialized knowledge of pharmacotherapy to direct patient care via in-person and telephone consults.
  • Provide high quality, evidence-based, pharmaceutical care services and recommendations using approved protocols in collaboration with other CCA health professionals, including recommending and monitoring patient drug therapy or lifestyle modifications based on information obtained through patient interviews, objective monitoring parameters, review of drug profiles and patient medical record, for a variety of cardiac disease states including, but not limited to, weight management, lipid management, smoking cessation, heart failure, cardiology-oncology, and structural heart interventions.
  • Identify, resolve, and prevent potential and actual medication-related problems including: untreated indications, improper drug selection, sub-therapeutic dosage, overdose, failure to receive medication, adverse drug reactions, drug interactions, and medication without indication.
  • Provide education to patients: counsel on adherence, medication mechanism of action, administration, monitoring requirements, and potential side effects.
  • Provide and disseminate verbal or written education to healthcare providers, nurses, and staff.
  • Interact with and mentor pharmacy students, as well as post-graduate pharmacy residents.
  • Provide clinical, pharmacy, and medication expertise in policy development, operations, and medication therapy.
  • Complete progress notes and provide documentation in electronic medical records.
