About The Position

Opening in early 2026, Riverside Smithfield Hospital will bring high-quality healthcare closer to home for Isle of Wight County residents. Located on a beautifully landscaped 27-acre medical campus at 19339 Benn’s Grant Boulevard in Smithfield, the new hospital will provide modern facilities and comprehensive medical services to meet the growing needs of the community. The 200,000-square-foot, three-story facility will offer a range of essential healthcare services including an Emergency Department, Diagnostic Imaging Services, 38 Medical/Surgical beds, 12 Intensive Care Unit (ICU) beds and 4 Operating Rooms. We anticipate start dates to begin Fall, 2025. Overview Supervises and performs compounding packaging and dispensing of pharmaceuticals according to prescription, physician order or floor stock request in compliance with state and federal law and regulation. Possesses the skills and ability to assess data reflective of patient status and to interpret this information and identify the patients' needs in relation to age specific requirements, and to provide care as described in accordance with state and federal law and regulation, and facility policy and procedures. Reviews, validates, monitors patient profiles to assure appropriate dosage and therapy as well as patient specific assessment of medication orders.

Requirements

• Doctoral Degree, Pharmacy (Required) or Bachelors Degree, Pharmacy (Required)
• Licensed Pharmacist - Virginia Department of Health Professions (VDHP) within 30 Days(Required)
• CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) (Preferred)
• Knowledge of contemporary pharmacy practice
• Up-to-date knowledge on medications
• Up-to-date knowledge on clinical practice guidelines related to medication therapy
• Up-to-date knowledge on state and federal law and regulation and regulatory standards related to practice setting
• Ability to assess patients related to medication therapy
• Analytical and problem solving skills
• Strong written and verbal communication skills
• Ability to work in a collaborative team environment

Responsibilities

• Reviews, validates, and assesses patient specific medication orders.
• Monitors patient profiles to assure appropriate dosage and therapy.
• Reviews alerts associated with verification and escalates concerns to provider or pharmacy leadership as appropriate.
• Supervises and performs compounding (both sterile and non-sterile), packaging, and dispensing of pharmaceuticals according to prescription, physician order, or floor stock request in compliance with state and federal law and regulation.
• Handles all aspects of pharmacy operations to include order review and validation, sterile and non-sterile compounding, controlled substance/diversion management, pharmacy automation, and all state/federal law and regulation.
• Establishes an environment of safety and assures safe practices for the patient as evidenced by recognizing and implementing safe medication practices, and following policies and procedures that comply with regulatory agency regulations.
• Responsible for all core clinical pharmacy programs and commitment to evidence-based patient care, to including, but not limited to IV to PO, renal/hepatic dosing, therapeutic interchange, antimicrobial stewardship, anticoagulation management, duplicate therapy, drug-drug interactions, non-formulary management and weight based dosing.
