About The Position

Alliance Cancer Specialists is seeking a highly motivated and detail-oriented Clinical Pharmacist to join our outpatient oncology team. This role is integral to ensuring safe, effective, and cost-efficient medication use for oncology patients, with a strong emphasis on formulary management, therapeutic drug substitutions, and oversight of oral chemotherapy dispensing.

Requirements

  • Doctor of Pharmacy (PharmD) degree required.
  • Completion of PGY1 residency; PGY2 in Oncology Pharmacy preferred.
  • Current pharmacist license in Pennsylvania.
  • Minimum 2 years of experience in oncology pharmacy practice, preferably in an outpatient setting.
  • Strong clinical knowledge of oncology therapeutics.
  • Excellent communication and collaboration skills.
  • Proficiency in EMR systems and pharmacy dispensing software.
  • Ability to manage multiple priorities in a fast-paced environment
  • Ability to stand and walk for extended periods and lift up to 40 lbs
  • Manual dexterity and visual acuity required for patient care tasks
  • Exposure to communicable diseases, toxic substances, and medical preparations common to oncology clinics
  • In-person interaction with patients, physicians, and clinical staff is essential

Nice To Haves

  • Board Certification in Oncology Pharmacy (BCOP) preferred.

Responsibilities

  • Maintain and update the oncology formulary in collaboration with the Pharmacy & Therapeutics Committee.
  • Evaluate new oncology medications and therapies for inclusion based on clinical efficacy, safety, and cost-effectiveness.
  • Monitor utilization trends and recommend changes to optimize formulary adherence.
  • Assess and implement therapeutic substitutions in alignment with institutional protocols and evidence-based guidelines.
  • Collaborate with oncologists and care teams to ensure appropriate substitutions that maintain therapeutic intent.
  • Educate staff and providers on substitution policies and rationale.
  • Oversee the dispensing of oral oncology agents, ensuring accuracy, safety, and compliance with regulatory standards.
  • Provide patient counseling on oral chemotherapy regimens, including adherence strategies and side effect management.
  • Monitor refill adherence and intervene as needed to support continuity of care.
  • Participate in multidisciplinary rounds and tumor boards as needed.
  • Provide drug information and clinical decision support to providers and nursing staff.
  • Assist in developing and updating treatment protocols and order sets.
  • Ensure compliance with USP standards, state and federal regulations, and institutional policies.
  • Participate in quality improvement initiatives related to oncology pharmacy services.
  • Document interventions and outcomes in the electronic health record.
