About The Position

Staff Pharmacist Position Summary The Staff Pharmacist is responsible for the pharmaceutical care of all hospital clients from infant to geriatric. They provide consultative services including: anticoagulation, drug information, formulary reviews, in services on pharmacology/disease state pharmacotherapy, pharmacokinetics to medical, nursing, and allied health professionals. This includes the selection of medications to disease state, monitoring of patient’s therapeutic outcomes, follow-up on medication errors and evaluation of adverse drug reactions. Provides prescribed medications, other pharmaceuticals and compounded medications according to professional standards and practices, hospital policies and directives, regulatory agencies, and Federal and State laws. Performs and supervises medication order entry, preparation of all sterile medications, narcotic dispensing and accounting, and dispenses and prepares all investigational medications.

Requirements

  • Currently licensed and in good standing as Pharmacist with the Board of Pharmacy in California.
  • A Doctor of Pharmacy degree or equivalent experience
  • A minimum of six months of experience in hospital pharmacy practice preferred
  • Familiar with unit dose, I.V. admixture and clinical pharmacy

Responsibilities

  • Distribution Functions – 20%
  • Responsible for compliance with all applicable policies, procedures, codes and standards of the facility.
  • Maintains patient confidentiality as per HIPPA regulations.
  • Reviews, interprets and clarifies all physician orders for appropriate dose, efficacy, safety drug interactions, and incompatibilities.
  • Supervises technician functions.
  • Is knowledgeable of technician responsibilities and their time commitments.
  • Checks accuracy of technician distribution functions.
  • Performs and supervises IV solutions compounding.
  • Maintains all logs and required reports.
  • Monitors monthly inspections of all nursing areas, pharmacy and areas of the facility where drugs are dispensed, administered and stocked.
  • Maintains and audits narcotic distribution records.
  • Informs physicians of standard solutions and doses of LVP and IVPB orders as appropriate.
  • Clinical Functions – 20%
  • Monitors appropriateness of dose, route, frequency, allergies, and medication interactions and intervenes when necessary.
  • Contacts physician for notification of potentially significant interactions.
  • Monitor all medications for formulary compliance.
  • Completes all assessment activities within time frame indicated in department policy.
  • Communicates appropriate assessment findings to other care team members.
  • Performs pharmacokinetic dosing adjustments based on the patient’s medical history, diagnosis, weight, age and laboratory tests.
  • Performs total parenteral nutrition dosing adjustments based on the patient’s medical history, diagnosis, weight, age and laboratory tests.
  • Initiates and documents clinical interventions and follows up on all interventions left from previous shifts.
  • Provide drug information to the facility’s professional staff and patients.
  • Suggests cost-effective therapeutic equivalents or conversion of intravenous therapy to oral therapy when appropriate.
  • Identifies and assesses suspected adverse drug reactions and medication errors.
  • Collect data and monitor quality clinical services of the department.
  • Education – 20%
  • Identifies educational needs of the pharmacy or nursing staff in order to improve patient care activities related to medication usage.
  • Maintains and develops clinical skills by attending conferences, seminars and reviewing current literature and periodicals.
  • C. Provides orientation and training for new staff members and ongoing staff education.
  • Collaborates with physicians and nursing staff to provide patient education as appropriate
  • Inventory Management – 20%
  • Controls drug purchasing costs by utilizing purchasing agreements established through the hospital.
  • Controls drug costs through formulary management, therapeutic streamlining programs, clinical interventions, and controlling outdates.
  • Monitors the procurement and receipt of medications and supplies and the maintenance of par levels.
  • Medication Storage – 20%
  • Ensures the appropriate medication storage requirements are met.
  • Ensure continuous monitoring of medication storage within the hospital.
