Staff Pharmacist Position Summary The Staff Pharmacist is responsible for the pharmaceutical care of all hospital clients from infant to geriatric. They provide consultative services including: anticoagulation, drug information, formulary reviews, in services on pharmacology/disease state pharmacotherapy, pharmacokinetics to medical, nursing, and allied health professionals. This includes the selection of medications to disease state, monitoring of patient’s therapeutic outcomes, follow-up on medication errors and evaluation of adverse drug reactions. Provides prescribed medications, other pharmaceuticals and compounded medications according to professional standards and practices, hospital policies and directives, regulatory agencies, and Federal and State laws. Performs and supervises medication order entry, preparation of all sterile medications, narcotic dispensing and accounting, and dispenses and prepares all investigational medications.
