Requirements

• Applicants must have a doctorate degree in pharmacy.
• Current license with the state of Board of Pharmacy.
• BCLS required.

Nice To Haves

• ACLS preferred.

Responsibilities

• Coordinate and provide pharmaceutical care for patients, collaborating with the interprofessional team to optimize safe, effective medication use.
• Review and verify orders, manage drug therapy protocols educates staff/patients, and supports quality and regulatory standards.
• Manage drug therapy protocol as part of interdisciplinary team.
• Review, interpret, and evaluate medication orders to identify, prevent, and resolve drug-related problems.
• Provide drug information and education to medical staff, healthcare professionals, patients, and pharmacy students.
• Document intervention and outcomes for drug therapy quality improvement.
• Verify medication orders for accuracy/completeness prior to entry.
• Check filled medication prepared by technician.
• Maintain proficiency with pharmacy automated systems and software.
• Manage patients in an outpatient anticoagulation clinic, including point-of-care testing, result evaluation, dosing, and scheduling.
• Participate in performance improvement and required continuing education.
