About The Position

Provides and compounds prescribed medications, drugs, IV mixtures, and other pharmaceutical according to professional standards and practices. Provides information and services necessary for safe and appropriate use of drugs to patients and families, and medical and nursing staff to achieve optimal patient outcomes.

Requirements

  • Bachelor's Degree in related field
  • Up to one year of post degree course work and/or an internship and less than one year of previous experience.
  • Minimum of 2 years experience in general pharmacy practice as a pharmacist or additional experience or education in a specialty area of pharmacy practice.
  • Prior Pharma experience.
  • Ability to work independently.
  • Pharmacist license

Responsibilities

  • Completes written records of dispensing for all medication including controlled substances and investigational drugs as required by policy and regulation.
  • Consults with patient and families on medication use, dangers, side effects and composition.
  • Develops and implements new policies and/or protocols for drug use or handling and/or treatment protocols for implementation or review in a specialty area of pharmacy practice.
  • Maintains responsibility for resolving medication dispensing system problems for one or more patient care areas.
  • Provides in-service education and/or orientation concerning drug use to health professionals when necessary or as appropriate.
  • Oversees and participates in maintaining and properly storing an adequate inventory of pharmacy items. Assures stock is rotated and expiration dates are checked. Obtains items not stocked or out of stock from authorized sources.
  • Participates in annual evaluations of RNs mixing chemo and Pharmacist; Performs annual evaluations of chemo RNs competencies as it relates to drug mixing and dispensing at all clinical sites.
  • Participates in training and continued competency activities of RNs if necessary. Provides in-service training to other health- care staff.
  • Prepares or supervises preparation and accurate dispensing of medication to outpatients, including unit dose dispensing, IV admixture, chemotherapy, TPN, investigational drugs, other sterile products and extemporaneous preparations.
  • Reviews patient profiles and adverse drug reactions relating to the new order or order changes. Maintains effective communication with physicians and nurses to ensure safe and appropriate use of drugs.
  • Reviews/interprets medication orders written by physicians or other authorized health care providers. Verifies orders for completeness, accuracy and appropriateness in accordance with pharmaceutical care standards and prescribed policies and procedures.
