Requirements

Doctor of Pharmacy (PharmD) degree
Kansas Pharmacist License

Responsibilities

Review and process prescriptions for LMH Health Patients, Employees, Medical Staff and their dependents.
Vaccinate patients under collaborative practice agreement.
Educate patients about appropriate medication use.
Facilitate transitions of care through the Meds to Beds Program.
Provide medication therapy management (MTM) services.
Serve as a drug information resource for prescribers regarding mediation use in the outpatient setting.
Engage in professional development, maintaining skills and knowledge of clinical standards of care in the outpatient setting.
Works the overnight shift.
