About The Position

The Pharmacotherapy Clinical Pharmacist is responsible for providing comprehensive pharmaceutical care to patients, focusing on optimizing medication therapy and improving health outcomes. The pharmacist collaborates closely with healthcare providers and patients to manage chronic diseases, ensure safe medication use, and promote patient education. This role requires a high level of clinical knowledge, communication skills, and patient-centered care.

Requirements

  • Licensed pharmacist in the state of Tennessee
  • completion of a PGY-1 Pharmacy Practice residency or a minimum of two (2) years of experience as a clinical pharmacist in an ambulatory care setting preferred.
  • One (1) to three (3) years of experience in a clinical setting.
  • Current state pharmacy license.

Responsibilities

  • Medication Therapy Management (MTM): Conduct comprehensive medication reviews for patients, optimizing therapy for chronic conditions such as diabetes, hypertension, cardiovascular diseases, asthma, COPD, and more. Provide initial clinical medication counseling and regular patient reassessments in person to ensure appropriateness of specialty therapy and ongoing therapy management.
  • Collaborative Practice: Work in collaboration with physicians, nurses, and other healthcare professionals to develop and implement individualized care plans. Work in partnership with providers and clinical staff to integrate pharmacy services and coordinate care with eligible patients.
  • Patient Education: Provide education to patients and caregivers regarding medication use, potential side effects, drug interactions, and lifestyle modifications to improve therapeutic outcomes.
  • Chronic Disease Management: Monitor and adjust medication regimens in accordance with clinical guidelines and patient progress. Initiate or modify therapy under collaborative practice agreements when appropriate.
  • Adherence Monitoring: Monitor patient adherence to prescribed medications, identify barriers to adherence, and develop strategies to improve compliance. Refer appropriate patients to financial counseling for copay assistance through manufacturer or charity programs.
  • Clinical Documentation: Maintain detailed patient records, documenting clinical interventions, outcomes, and any changes to therapy.
  • Consultation Services: Serve as a consultant to healthcare providers on medication-related issues, including drug selection, dosage adjustments, and management of side effects or drug interactions. Enroll specialty pharmacy patients into medication management programs by assisting with recruitment of patients in clinic.
  • Laboratory Monitoring: Review and interpret laboratory results (e.g., blood glucose, INR, cholesterol levels) to make informed decisions about medication adjustments.
  • Quality Improvement: Participate in clinic-based quality improvement projects, focusing on medication use and clinical outcomes.
  • Continuing Education & Training: Stay current with the latest clinical guidelines, research, and best practices in pharmacotherapy. Provide education to other healthcare team members when necessary. Participate in ongoing trainings related to role, including but not limited to disease state overviews, clinical guidelines, pharmacology, REMS programs, etc.
  • Communication: Communicate and represent pharmacy effectively throughout all levels of the organization internally and to all external partners. Communicate with physicians and other clinicians as necessary to facilitate exemplary service for new and existing specialty pharmacy patients.
  • Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives and participates in quality improvement initiatives as requested.
