About The Position

This position requires the Pharmacist to ensure the safe and appropriate distribution and use of pharmaceuticals throughout the facility and to provide information about medications to the institution's staff and patients. This function includes a program of Quality Improvement designed to monitor and assure the quality of drug use. This function also includes a program of Performance Improvement designed to monitor and assure the safe use of medications throughout the facility.

Requirements

  • Bachelor's Degree in Pharmacy or PharmD
  • New York State Pharmacy License
  • Current New York State Pharmacist Registration
  • Working knowledge of pharmacy dispensing systems and sterile compounding, including cytotoxic and other hazardous agents

Responsibilities

  • To have a thorough knowledge of the hospital's drug formulary and all policies and procedures governing the procurement, distribution, and control of pharmaceuticals for inpatients and outpatients.
  • To monitor and check for drug/drug and drug/food interactions, allergies, correct dosages and drug/disease interactions.
  • To ensure that patients receive the correct drugs at the proper time through proper packaging, labeling, distribution and by monitoring the nursing medication administration record (MAR).
  • To prepare sterile drug products in the laminar air flow hood in accordance with pharmacy policies and procedures.
  • To routinely inspect for proper medication storage in the hospital including the removal of drugs prior to their expiration date, the removal of recalled drugs, and to ensure the security of controlled drugs.
  • To supply and maintain all drugs for use throughout the hospital as floor stock, on crash carts, and, by emergency rescue squads.
  • To provide the hospital and medical staff and patients with accurate and relevant information about drugs and their use.
  • To review and approve the Pharmacy Assistant/Technicians' activities pertaining to the dispensing of drugs which are mechanical in nature, and which do not involve the exercise of professional judgment.
  • To accurately dispense and inventory all controlled substances daily according to federal and state laws.
  • To maintain and document a program of continuing education.
  • To participate in taking an annual inventory of all drugs and any special inventory when required.
  • All other duties as assigned by the Director of Pharmacy.
  • Maintain order and cleanliness.
  • Cooperates with all personnel in the proper conduct of the department.
  • Rotates within the department and by shift as required.
  • Maintains ethical relationships.
  • Adheres to hospital and departmental rules and regulations.
  • Participate in in-service programs.
